With its partners, StayWell Health Management and the Medicare program, the Aging & Disablity Resource Center of Broward County (ADRC) is operating a demonstration program to help randomly selected beneficiaries learn about their health risks and take steps to better manage their health.
Under the program name, "Staying Healthy USA," StayWell provides health questionaires, tailored health reports, customized educational materials, health counseling, self-management tools and referrals to the ADRC for demonstration participants. The programs will run through April 30, 2012.
Since the selection is made by StayWell, no one can register online.
