The Community Living Program is designed to provide needed in-home help immediately after a hospital or temporary nursing home stay, as well as other crises situations.

Community Living Program

Project Purpose

The Community Living Program is a partnership between the U.S. Administration on Aging, the Florida Department of Elder Affairs, the Aging and Disability Resource Center of Broward County, and local partnering agencies.

The primary goal of the project is to allow the client to remain at home for as long as possible, thereby avoiding premature, long-term nursing home placement.

This project is designed to provide needed in-home help immediately after a hospital or temporary nursing home stay, as well as other crises situations.

Eligible clients will receive up to 45 days of in-home services while sharing part of this cost. After the 45-day period, if needed, continuity of care services may be available.

Project Eligibility

To participate in this program, eligible clients must be:

  • Residents of Broward County.
  • At least 60 years old.
  • Have private health insurance or Medicare, and not Medicaid.
  • Have a monthly income minimum of $700 and a maximum of $4,100 per individual and/or minimum income of $900 and a maximum of $5,500 per couple.
  • Have assets at a minimum of $25,000, with a maximum cap of $179,999 per individual and/or a minimum of $25,000, with a maximum cap of $249,999 per couple.
  • Preparing for discharge from the hospital or from a temporary nursing home stay.
  • Not receiving hospice services.
  • In the community in need of crisis resolving or short-term, in-home services to avoid nursing home placement and spend-down to Medicaid.

The Program:

  • Prevents premature or long-term nursing home admission and spend-down to Medicaid financial eligibility.
  • Provides short-term services for up to a 45-day period, with a share of cost paid by the client, to promote a safe home environment.
  • Provides individualized supportive services to meet the needs of clients and their caregivers, such as:
    • Case Management
    • Caregiver Respite
    • Medication Management
    • Personal Care
    • Transportation
    • Other Needed Services Relating to the Program
  • Provides access to other resources through the Aging and Disability Resource Center, where experienced and knowledgeable Bilingual Staff will help clients find answers to their questions.
